Why this applies to Fomoterol

Fomoterol falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Schisandra interacts with.

Theoretically, schisandra might increase the levels and clinical effects of CYP2C19 substrates.
In vitro research shows that schisandra inhibits CYP2C19, and animal research shows that schisandra increases the concentration of voriconazole, a CYP2C19 substrate. Theoretically, schisandra may also inhibit the metabolism of other CYP2C19 substrates. This effect has not been reported in humans.

Here's the honest picture for formoterol (Foradil). The concern on file is that schisandra may slow down a liver enzyme called CYP2C19, which in theory could let drugs cleared by that enzyme build up a bit. But formoterol isn't really a CYP2C19 drug. Your body mostly clears it through a different process (glucuronidation and CYP2D6), so this particular interaction is a poor fit for it.

On top of that, the evidence here is only from lab and animal studies, never shown in people. If formoterol levels did rise, you might notice more shakiness, a faster or pounding heartbeat, or jitteriness. That's unlikely here, but since formoterol affects the heart, it's worth a quick chat with your pharmacist or doctor before combining them.

Here's the gist with formoterol (Foradil) and schisandra. Your inhaler relaxes the muscles around your airways so you can breathe easier with COPD. The concern on file is that schisandra may speed up a liver enzyme called CYP2C9, which could in theory clear formoterol from your body a little faster. If that happened, you might notice your inhaler not working as well, meaning more wheezing, shortness of breath, or chest tightness.

That said, this is only a theoretical worry from lab and animal studies, and it has never actually been shown in people. So I wouldn't lose sleep over it. Still, since breathing meds are important to keep working, mention the schisandra to your pharmacist or doctor before adding it.

What Is Formoterol Oral Inhalation Used For?

Brand name: Foradil

Formoterol fumarate inhalation solution is used for the long-term maintenance treatment of airway narrowing (bronchoconstriction) in adults with COPD, including chronic bronchitis and emphysema. It is taken twice daily — morning and evening — as a regular, ongoing treatment.

Important limits: This medication is not for relieving sudden COPD flare-ups or breathlessness in the moment, and it is not approved to treat asthma. A separate short-acting inhaler should always be on hand for sudden symptoms.
