Opicapone falls under P-GLYCOPROTEIN SUBSTRATES — the group Schisandra interacts with.

Schisandra might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that schisandra extracts and constituents such as schisandrin B inhibit P-glycoprotein mediated efflux in intestinal cells and in P-glycoprotein over-expressing cell lines. Additionally, a small clinical study shows that schisandra increases the peak concentration and overall exposure to talinolol, a P-glycoprotein probe substrate. Theoretically, schisandra might inhibit the efflux of other P-glycoprotein substrates.

Here's the gist for Opicapone (Ongentys), which you take alongside your levodopa/carbidopa for Parkinson's. Opicapone is partly handled by a tiny pump in your gut called P-glycoprotein that helps move drugs back out. Schisandra appears to block that pump, so in theory a bit more opicapone could get absorbed and stay in your system, nudging its effects upward.

If that happened, you might notice more of opicapone's known effects, like extra dyskinesia (involuntary movements), dizziness, low blood pressure on standing, or trouble sleeping, since it boosts your levodopa. I want to be honest though: this concern comes mostly from lab and test-tube work, so it's a maybe, not a certainty. Before adding schisandra, please run it by your pharmacist or neurologist so they can keep an eye on things.

Ongentys (opicapone) is used alongside levodopa/carbidopa to treat Parkinson's disease in adults who experience 'off' episodes — periods when medication wears off and Parkinson's symptoms return before the next dose is due.

It is not used alone. Ongentys works as an add-on to help levodopa stay active longer, reducing the time patients spend in these 'off' states.
