Selenium may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Clinical research suggests that taking selenium 10 mcg/kg/day can increase bleeding times by increasing prostacyclin production, which inhibits platelet activity. Other clinical research suggests that taking selenium 75 mcg daily, in combination with ascorbic acid 600 mg, alpha-tocopherol 300 mg, and beta-carotene 27 mg, reduces platelet aggregation.

Here's the situation with selenium and Defitelio (defibrotide). Defibrotide is a clot-preventing medicine given in the hospital to help blood flow through the liver after a stem-cell transplant. On its own, it already raises your bleeding risk. Selenium, in higher supplement doses, may also make platelets a little less sticky, so stacking the two together could, in theory, push that bleeding risk up a bit further. Think easier bruising, nosebleeds, or bleeding that takes longer to stop.

I want to be honest that this concern comes from lab and early human studies, not strong proof, and the food amounts of selenium you'd get from a normal diet are very unlikely to matter. Since defibrotide is only used in a closely watched hospital setting, let your transplant team know about any supplements you take.

What Is Defibrotide Injection Used For?

Brand name: Defitelio

Defitelio (defibrotide injection) is used to treat hepatic veno-occlusive disease (VOD) — also called sinusoidal obstruction syndrome (SOS) — in adult and pediatric patients. This is a serious condition where small blood vessels in the liver become blocked, most often after a hematopoietic stem-cell transplant (HSCT).

Defitelio is specifically indicated when VOD is accompanied by kidney dysfunction or pulmonary (lung) dysfunction, making it a treatment for one of the most severe forms of this complication.
