Isoniazid, Pyrazinamide, Rifampin falls under PHOTOSENSITIZING DRUGS — the group St. John's Wort interacts with.
St. John's wort might increase photosensitivity reactions due to its hypericin content. However, some clinical research shows that very high doses of St. John's wort are needed to produce phototoxicity in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to know about pairing St. John's wort with your TB combination (Rifater, which contains isoniazid, pyrazinamide, and rifampin). St. John's wort carries a compound called hypericin that can make your skin a little more sensitive to sunlight. Some of these TB medicines can do the same thing on their own, so stacking them together could, in theory, raise your odds of a sunburn-like reaction, redness, or a rash on sun-exposed skin.
I'll be honest with you, this is a theoretical concern based on how these things behave, not strong human proof, and research suggests it usually takes very high doses of St. John's wort to trigger phototoxicity. Still, with TB treatment being so important, please loop in your pharmacist or doctor before combining them, and use sunscreen if you do go outside.
