Theoretically, St. John's wort might inhibit reuptake and increase levels of serotonin, resulting in additive effects with serotonergic drugs.
Concomitant use of serotonergic drugs with St. John's wort can lead to increased adverse effects and increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstriction disorders.

Here's the thing to keep in mind: both venlafaxine (Effexor) and St. John's wort push serotonin levels up in your brain. Venlafaxine does it as an SNRI, and the herb appears to nudge serotonin in the same direction. Stacking two serotonin-raising agents can be too much of a good thing.

When serotonin climbs too high, you can run into what's called serotonin syndrome. Watch for things like agitation, a racing heart, shivering or muscle twitching, sweating, fever, or feeling confused. This is mostly a theoretical concern based on how each one works, not a flood of reported cases, so don't panic. Still, because the risk is real enough, please check with your pharmacist or doctor before combining them rather than starting or stopping anything on your own.

Brand names: Effexor XR, Effexor

Venlafaxine is used to treat major depressive disorder (MDD) across all available formulations. Extended-release capsules (including Effexor XR) are also approved for generalized anxiety disorder (GAD), social anxiety disorder (SAD), and panic disorder (PD).

Extended-release tablets are approved for major depressive disorder and social anxiety disorder. Immediate-release venlafaxine tablets are indicated for major depressive disorder only.
