Strontium and Magaldrate

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTACIDS

Why this applies to Magaldrate

Magaldrate falls under ANTACIDS — the group Strontium interacts with.

Antacids can reduce the absorption of strontium, potentially decreasing its effectiveness.
Aluminum hydroxide and magnesium hydroxide can reduce absorption of strontium ranelate by 20% to 25%. Calcium can reduce its absorption to 60% to 70%. Avoid taking antacids within 2 hours of a dose of strontium ranelate. This interaction has also been reported with orally-administered radioactive strontium-85. It will likely also occur with other forms of strontium.
Severity Moderate Likelihood Probable Evidence C

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: Magaldrate (Riopan) is an antacid that combines aluminum and magnesium to soothe heartburn and indigestion. When it shares your stomach with a strontium supplement, it can grab onto the strontium and keep your body from absorbing it well. Studies with aluminum and magnesium antacids show strontium absorption dropping by roughly 20 to 25 percent.

So this isn't about Magaldrate becoming dangerous. It's that your strontium may not work as well for your bones if the two are taken too close together. The easy fix most experts agree on is spacing them out by at least 2 hours. If you take both regularly, run your timing past your pharmacist or doctor first.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

This page covers Strontium only. If you take other supplements — a multivitamin, a sleep aid, something for joints — the Magaldrate page lists 30 supplements it has a documented interaction with, worst first, each one explained.

The most serious there right now: Activated Charcoal (major), Cinchona (moderate), Coffee Charcoal (moderate), European Mandrake (moderate) and Glucomannan (moderate).

By severity: 1 major · 10 moderate · 19 minor.

From our pharmacist’s overview on the Magaldrate page

“Activated charcoal carries the only major rating among the 30 supplements and herbs on file for magaldrate (Riopan), because it adsorbs medicines in the gut and cuts how much the body absorbs. That single entry is why this page deserves real care even though magaldrate is just a heartburn remedy.”

About Strontium

What is Strontium?

  • Bone health and osteoporosis
  • Reducing fracture risk
  • Tooth sensitivity (in some toothpastes)
  • Supporting bone density

Strontium is a mineral that has been studied mainly for bone health and osteoporosis. The prescription form (strontium ranelate) showed benefit for fracture risk but carries serious heart-related safety concerns, while the over-the-counter supplement form (strontium citrate) has much less evidence. Talk to your doctor or pharmacist before using it, especially if you have heart disease or are at risk for blood clots.
