Strontium and Aluminum Hydroxide

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTACIDS

Why this applies to Aluminum Hydroxide

Aluminum Hydroxide falls under ANTACIDS — the group Strontium interacts with.

Antacids can reduce the absorption of strontium, potentially decreasing its effectiveness.
Aluminum hydroxide and magnesium hydroxide can reduce absorption of strontium ranelate by 20% to 25%. Calcium can reduce its absorption to 60% to 70%. Avoid taking antacids within 2 hours of a dose of strontium ranelate. This interaction has also been reported with orally-administered radioactive strontium-85. It will likely also occur with other forms of strontium.
Severity Moderate Likelihood Probable Evidence C

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: if you take an aluminum hydroxide antacid like Amphojel or Gaviscon too close to your strontium, the antacid can block some of the strontium from being absorbed. Studies show this can cut strontium absorption by roughly 20% to 25%, so you'd basically get less benefit from the strontium you paid for. This isn't a safety danger, it just means the strontium may not work as well for your bones.

The easy fix most experts agree on is timing. Try to separate them by at least 2 hours. That way your antacid can still do its job calming heartburn or stomach acid, and your strontium gets a fair shot at being absorbed. If you're unsure how to space your doses, your pharmacist can map it out with you.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.

From our pharmacist’s overview on the Aluminum Hydroxide page

“An antacid sounds harmless, and that is exactly why the supplement interactions with aluminum hydroxide (Amphojel, Gaviscon) deserve real attention before you combine it with much of anything. The most serious entry is activated charcoal, which can soak up many medications in the gut and sharply cut how much of them your body actually…”

About Strontium

What is Strontium?

  • Bone health and osteoporosis
  • Reducing fracture risk
  • Tooth sensitivity (in some toothpastes)
  • Supporting bone density

Strontium is a mineral that has been studied mainly for bone health and osteoporosis. The prescription form (strontium ranelate) showed benefit for fracture risk but carries serious heart-related safety concerns, while the over-the-counter supplement form (strontium citrate) has much less evidence. Talk to your doctor or pharmacist before using it, especially if you have heart disease or are at risk for blood clots.
