Enfortumab Vedotin-Ejfv falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sweet Annie interacts with.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the honest picture with Padcev (enfortumab vedotin). This is an antibody-drug conjugate, and after it docks onto cancer cells it releases a chemo payload called MMAE, which is partly handled by the liver enzyme CYP3A4. Sweet Annie may nudge that same enzyme, but the lab data pull in two directions: one part (artemisinin) seems to speed it up, while the whole extract may slow it down. So we honestly can't predict which way the scale tips.

If MMAE levels rose, side effects like numbness or tingling in the hands and feet, rash, fatigue, nausea, or high blood sugar could become more bothersome. If levels dropped, the treatment might work less well. Since this is only test-tube evidence and Padcev is serious cancer therapy, please run any Sweet Annie use past your oncology team first.

What Is Enfortumab vedotin-ejfv Injection Used For?

Brand name: Padcev

  • Enfortumab vedotin-ejfv injection is used to treat urothelial cancer (cancer of the lining of the bladder and other parts of the urinary tract) that has spread to nearby tissues or other parts of the body or cannot be removed by surgery, and has worsened after treatment with other chemotherapy medications or if these chemotherapy medications cannot be used for treatment. Enfortumab vedotin-ejfv injection is in a class of medications called monoclonal antibodies. It works by helping your immune system to slow or stop the growth of cancer cells.
