Here's the simple version: both Sweet Annie and Fostamatinib (Tavalisse) can be hard on your liver. Fostamatinib is already known to raise liver enzymes in some people, and there's a concern that Sweet Annie may stress the liver too. Put them together and you could, in theory, add those effects up, which might increase your chance of liver irritation.

I want to be honest about the strength of this: it's based on weak, anecdotal evidence, not solid studies, so it's a caution rather than a sure thing. Watch for warning signs like yellowing skin or eyes, dark urine, belly pain on the right side, nausea, or unusual tiredness. Please don't start or stop anything on your own. Check with your pharmacist or doctor first, since they may already be monitoring your liver labs on this medication.

Fostamatinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the thing worth knowing about Fostamatinib (Tavalisse). Once you swallow it, your body converts it into its active form, called R406, and that active piece is then broken down mainly by a liver enzyme called CYP3A4. Sweet Annie may bump up or slow down that same enzyme, so in theory it could shift how much active drug stays in your system.

I want to be honest about the evidence: this is based only on lab (test-tube) studies, and the results actually point in both directions, so we don't really know what happens in a real person. If your levels rose, you might see more side effects like diarrhea, high blood pressure, or nausea. If they dropped, your platelet support could weaken. Nothing here means you need to stop either one. Just loop in your pharmacist or doctor first.
