Here's the simple version: both Sweet Annie and your myeloma medication Ninlaro (ixazomib) can put a little extra stress on your liver. The worry is that stacking them together could add up and bother your liver more than either one alone. This isn't about Sweet Annie changing how ixazomib is broken down or making it stronger or weaker. It's just about two things that can each be hard on the liver being used at the same time.

I want to be honest that this concern is theoretical and based mostly on anecdotal reports, not solid studies, so don't panic. Still, since ixazomib already requires liver monitoring, watch for things like yellowing of the skin or eyes, dark urine, belly pain, or unusual tiredness, and please run any Sweet Annie use past your oncology team or pharmacist first.

Ixazomib Citrate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the situation with Ninlaro (ixazomib) and Sweet Annie. Your body relies on a liver enzyme called CYP3A4 to clear ixazomib. The artemisinin in Sweet Annie may nudge that enzyme in different directions depending on which part of the plant you're looking at. In lab studies, purified artemisinin sped the enzyme up (which could clear ixazomib faster and weaken its cancer-fighting punch), while whole-plant extract slowed it down (which could let ixazomib build up and worsen side effects like nausea, diarrhea, low blood counts, or nerve tingling).

Honestly, this is only test-tube evidence, so we don't really know what happens in a real person. Since ixazomib is a serious cancer therapy, please don't add Sweet Annie without first clearing it with your oncologist or pharmacist.

What Is Ixazomib Used For?

Brand name: Ninlaro

Ninlaro (ixazomib) is used to treat multiple myeloma — a cancer of plasma cells in the bone marrow. It is not used alone; it is always taken together with two other medicines, lenalidomide and dexamethasone. This combination is for adults who have already received at least one prior treatment for their myeloma.

Ninlaro is not recommended for use as maintenance therapy or in newly diagnosed multiple myeloma patients on a lenalidomide and dexamethasone regimen outside of controlled clinical trials.
