Why this applies to Zonisamide

Zonisamide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sweet Annie interacts with.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Your body relies on a liver enzyme called CYP3A4 to help clear zonisamide from your system. Sweet Annie may bump into this same pathway, but here is the catch: the lab data point in two different directions. One compound in the plant (artemisinin) seemed to speed the enzyme up, while the whole-plant extract appeared to slow it down. So we honestly cannot say which way it would push your zonisamide levels.

If levels dropped, your seizure control could slip. If they rose, you might notice more drowsiness, dizziness, trouble concentrating, or appetite changes. The good news is this is only test-tube evidence, not real-world cases, so it is far from proven. Still, with a seizure medicine I would not guess. Please check with your pharmacist or doctor before adding Sweet Annie.

What Is Zonisamide Used For?

Brand names: Zonegran, Zonisade

Zonisamide is used as add-on therapy — meaning it is combined with other seizure medications, not used alone — to treat partial seizures (seizures that start in one part of the brain) in adults with epilepsy. Zonegran (capsule) and generic zonisamide capsules are approved for adults with epilepsy.

Zonisade (oral suspension) is approved as add-on therapy for partial-onset seizures in adults and in patients 16 years of age and older.
