Why this applies to Effient

Effient falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Tangerine interacts with.

In vitro, tangeretin, a constituent of tangerine, induces a 52% increase in the metabolism of midazolam by cytochrome P450 3A4 (CYP3A4). This suggests that tangeretin may stimulate CYP3A4 activity. However, in humans, drinking tangerine juice 200 mL slightly delayed the absorption, but did not affect the metabolism, of midazolam, a CYP3A4 substrate. Theoretically, tangerine juice might increase CYP3A4 activity and decrease levels of drugs metabolized by this enzyme. However, this effect is unlikely.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone, Orasone), sertraline (Zoloft), sibutramine (Meridia), and many others.

Here's the good news first: tangerines and Effient (prasugrel) really shouldn't cause you any trouble. Effient is what we call a prodrug, which means your body has to switch it on before it can thin your blood and prevent clots. While CYP3A4 plays a role in that activation, the concern with tangerine is mostly theoretical. In lab dishes a tangerine compound sped up this enzyme, but when researchers actually tested tangerine juice in people, it didn't change how the enzyme worked.

So in plain terms, the worry that tangerine could nudge your Effient levels off is rated minor and unlikely, and food amounts of tangerine are not something I'd lose sleep over. Keep enjoying them. If you ever notice unusual bruising, bleeding, or new chest symptoms, that's worth a quick call to your doctor or pharmacist regardless.

What Is Prasugrel Used For?

Brand name: Prasugrel

Prasugrel tablets (brand names Effient and Prasugrel) are used to lower the risk of dangerous blood clots — including clots that can block a stent — in people with acute coronary syndrome (ACS), which includes unstable angina (chest pain at rest) and heart attacks (both NSTEMI and STEMI types). It is used specifically in patients who are being treated with a procedure called percutaneous coronary intervention (PCI), in which a catheter is used to open a blocked coronary artery, often with a stent.

Effient has been shown to reduce the combined rate of cardiovascular death, nonfatal heart attack, or nonfatal stroke compared to clopidogrel — with the benefit driven mainly by fewer heart attacks.
