HEPATOTOXIC DRUGS
Why this applies to Ripretinib
Ripretinib falls under HEPATOTOXIC DRUGS — the group Testolone interacts with.
In a case report, a 52-year-old male was diagnosed with liver injury thought to be related to the use of products containing testolone. Also, elevated liver enzymes were commonly reported in a dose escalation study of testolone.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with pairing Testolone (RAD-140) and ripretinib (Qinlock): both can be hard on your liver. Ripretinib by itself can raise liver enzymes and, in some people, cause more serious liver problems. Testolone has been tied to liver injury in a case report and to elevated liver enzymes in early studies. Stacking two liver stressors together may add up, which is the worry here.
This is mostly a theoretical, additive risk rather than one drug changing how the other works, so I don't want to overstate it. But keep in mind Testolone is an unapproved, illegally sold SARM with real safety concerns. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual fatigue, and please talk with your oncologist or pharmacist before combining them.

Every interaction above is graded three ways — here’s the key to each label.
Clinically significant. Avoid the combination, or use only under close professional supervision.
Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.
Generally not serious. Be aware of it and mention it to your pharmacist.
- LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Ripretinib page
“If you take ripretinib (Qinlock) for a GI stromal tumor, check this list carefully, since six of its 168 entries rate major. Ripretinib is cleared by the liver enzyme CYP3A4, and 121 of these ingredients can push its blood level up or drag it down through that pathway.”
