Efgartigimod alfa-fcab falls under IMMUNOSUPPRESSANTS — the group Thuja interacts with.

Thuja might stimulate immune function. Theoretically, taking thuja might decrease the effects of immunosuppressive therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), and other corticosteroids (glucocorticoids).

Here's the thinking on combining Thuja with Vyvgart (efgartigimod). Vyvgart works for myasthenia gravis by lowering a certain type of antibody (IgG), which calms the immune attack on your muscles. Thuja, on the other hand, is thought to stimulate immune activity. So the worry is that Thuja could theoretically push in the opposite direction and make Vyvgart work a little less well, meaning your muscle weakness, drooping eyelids, or fatigue might not improve as much.

I'll be straight with you though: this is a theoretical concern based on how each one behaves, not something proven in people taking these two together. Vyvgart isn't a prodrug, and Thuja doesn't change how it's processed. Still, since Thuja's undiluted oil can be toxic and the benefit is unproven, please check with your neurologist or pharmacist before adding it.
