Naldemedine Tosylate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Thunder God Vine interacts with.
In vitro research shows that triptolide, a constituent of thunder god vine, inhibits CYP3A4 enzymes in human microsomes. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Symproic (naldemedine), which your body clears using a liver enzyme called CYP3A4. Lab studies suggest a compound in thunder god vine, triptolide, can slow that enzyme down. If it does, naldemedine could build up a bit more than usual in your system, which in theory means stronger drug effects and a higher chance of side effects.
For naldemedine, that mostly means more belly trouble: stomach pain, diarrhea, nausea, or cramping. The important part is that this is theoretical, coming from test-tube work and never actually reported in people. So I wouldn't panic. Still, thunder god vine can be toxic on its own, so please loop in your pharmacist or doctor before combining them.
