Ranolazine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Thunder God Vine interacts with.

Theoretically, thunder god vine might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that triptolide, a constituent of thunder god vine, inhibits CYP3A4 enzymes in human microsomes. This interaction has not been reported in humans.

Here's the gist for Ranexa (ranolazine). Your body relies on an enzyme called CYP3A4 to clear this drug out of your system. Thunder God Vine contains a compound (triptolide) that may slow that same enzyme down. If clearance slows, ranolazine levels could climb higher than intended, which can mean more dizziness, nausea, constipation, headache, or, importantly, changes to your heart's rhythm (a longer QT interval).

I want to be honest about how solid this is: it's theoretical, seen only in lab dishes, never actually reported in real patients. So don't panic. Still, ranolazine is sensitive to CYP3A4 changes, so please loop in your doctor or pharmacist before combining these. Don't change anything on your own.

What Is Ranolazine Used For?

Ranolazine treats chronic angina — the recurring chest pain or pressure that happens when the heart isn't getting enough blood and oxygen. It is used when other angina medicines aren't enough on their own.

Both the extended-release tablet (sold as Ranexa and generic ranolazine) and the oral powder form (sold as Aspruzyo Sprinkle) are approved for this use. Ranolazine can be combined with other heart medicines such as beta-blockers, nitrates, calcium channel blockers, anti-platelet drugs, statins, ACE inhibitors, and angiotensin receptor blockers.
