Here is the interaction we found between these two, with the evidence behind it.
Oral Drugs
Why this applies to Epoprostenol
Epoprostenol falls under Oral Drugs — the group Thyroid Extract interacts with.
Well-controlled human studies have demonstrated the likely existence of this interaction.
Here's the thing worth knowing about this pairing: the interaction on file is really about how thyroid extract gets absorbed, not about epoprostenol (Flolan) itself. Epoprostenol is given as a continuous IV infusion through a pump, so it skips your digestive tract entirely. That means it won't bind up or block the thyroid hormone the way oral pills, antacids, calcium, or iron can.
Because of that, I wouldn't expect a meaningful clash between these two. The classic advice still applies to your thyroid product: take it on an empty stomach, away from other oral meds and minerals. Epoprostenol carries its own watch-points (flushing, headache, jaw pain, low blood pressure), so always loop in your doctor or pharmacist before adding any thyroid supplement.

Clinically significant. Avoid the combination, or use only under close professional supervision.
Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.
Generally not serious. Be aware of it and mention it to your pharmacist.

From our pharmacist’s overview on the Epoprostenol page
“American hellebore tops my concerns for anyone on epoprostenol (Flolan), the continuous infusion that opens blood vessels in the lungs to treat pulmonary arterial hypertension. The herb lowers blood pressure even in small doses, and stacked on a medicine that already relaxes those vessels, the drop can be dangerous.”

About Thyroid Extract
What is Thyroid Extract?
- Hypothyroidism (underactive thyroid)
- Fatigue and low energy
- Weight management
- Sluggish metabolism
- General thyroid support
Thyroid extract supplements are made from dried animal thyroid gland and may contain real thyroid hormones (T3 and T4), but the amounts in over-the-counter products are not standardized and can be unpredictable. Self-treating a thyroid problem with these supplements can be risky, so always work with a doctor or pharmacist and use prescription thyroid medicine when treatment is needed.
About Epoprostenol
What Is Epoprostenol Used For?
Brand name: Flolan
Epoprostenol treats pulmonary arterial hypertension (PAH) (WHO Group 1), which is high blood pressure in the arteries of the lungs. It is used to improve exercise capacity.
Both Veletri and Epoprostenol for injection are approved for this use. Studies included mostly people with NYHA Functional Class III–IV symptoms (moderate to severe limits on activity). The causes were mainly idiopathic or heritable PAH, or PAH linked to connective tissue diseases.

Epoprostenol Is Linked to Lower Levels of One Nutrient
Natural Medicines records Epoprostenol as a medicine that can lower these. This is not an interaction with Thyroid Extract — it is a separate effect of the medicine, and it is a reason some people are advised to supplement rather than to avoid one:
