Here the concern runs the other direction from what you might expect. Your cough and congestion medicine (the dextromethorphan and phenylephrine combo) isn't really the one being affected. Instead, taking it at the same time as a thyroid extract can get in the way of how well your body absorbs the thyroid hormone. The interaction on file is rated minor, but it's a real and well-documented one.

The simple fix is timing. Thyroid products work best on an empty stomach, so it's smart to keep at least 30 to 60 minutes between your thyroid dose and this cough syrup. That way the thyroid hormone gets absorbed properly. Since over-the-counter thyroid extract isn't standardized, I'd really encourage you to loop in your doctor or pharmacist about using it at all.
