Procainamide falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here the concern runs in one direction: your procainamide can affect how well your body absorbs the thyroid extract, not the other way around. When you swallow both pills together, the procainamide and other contents in your stomach can bind up some of the thyroid hormone, so less of it gets into your bloodstream. This isn't dangerous, just a timing issue, which is why this is rated a minor interaction.

The simple fix is spacing them out. Thyroid hormone works best on an empty stomach, so take it by itself and wait at least 30 to 60 minutes before your procainamide or food. Since unregulated thyroid extract doses are unpredictable to begin with, please loop in your pharmacist or doctor to sort out the safest plan.

About Procainamide

What Is Procainamide Used For?

Brand names: Pronestyl, Procanbid, Procan SR, Procan, Pronestyl-SR

Procainamide hydrochloride injection is used to treat documented, life-threatening ventricular arrhythmias — most notably sustained ventricular tachycardia (a dangerously fast heart rhythm originating in the lower chambers of the heart). Because of its serious risk profile, it is reserved for situations where the benefit clearly outweighs the risk.

Procainamide is not recommended for minor or asymptomatic arrhythmias, and treatment should always be started in a hospital setting where close monitoring is possible. Antiarrhythmic drugs, including procainamide, have not been shown to improve survival in patients with arrhythmias that are not life-threatening.
