Procainamide falls under QT INTERVAL-PROLONGING DRUGS — the group Sida Cordifolia interacts with.

Theoretically, Sida cordifolia might increase the risk of additive QT interval prolongation when taken with QT interval-prolonging drugs.
Sida cordifolia contains ephedrine. Clinical research shows that ephedrine from another herb, ephedra, can prolong the QT interval.

Here's the concern with pairing these two. Procainamide already has a known effect on your heart's electrical timing, lengthening what we call the QT interval. Sida cordifolia contains ephedrine, a stimulant that can do the same thing. Stacked together, those two effects may add up and raise the risk of a dangerous, irregular heartbeat (one type is called torsades de pointes).

Procainamide is a narrow-margin heart rhythm drug, so even small added stress on your heart's rhythm matters more than it would with an everyday medication. This particular warning is partly theoretical, drawn from ephedra research, but given Procainamide's risks I'd treat it seriously. Please don't start or stop anything on your own. Talk with your pharmacist or cardiologist first.

What Is Procainamide Used For?

Brand names: Pronestyl, Procanbid, Procan SR, Procan, Pronestyl-SR

Procainamide hydrochloride injection is used to treat documented, life-threatening ventricular arrhythmias — most notably sustained ventricular tachycardia (a dangerously fast heart rhythm originating in the lower chambers of the heart). Because of its serious risk profile, it is reserved for situations where the benefit clearly outweighs the risk.

Procainamide is not recommended for minor or asymptomatic arrhythmias, and treatment should always be started in a hospital setting where close monitoring is possible. Antiarrhythmic drugs, including procainamide, have not been shown to improve survival in patients with arrhythmias that are not life-threatening.
