Phenobarbital, Sodium Nitrate falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here's the simple version: this interaction is really about your thyroid extract, not so much the phenobarbital. When you swallow other pills, food, or minerals around the same time as thyroid hormone, they can grab onto the thyroid hormone in your gut and keep your body from absorbing all of it. That means your thyroid supplement may work less reliably if you take everything together.

The easy fix most people use is timing. Thyroid hormone absorbs best on an empty stomach, so taking it well apart (often first thing in the morning, 30 to 60 minutes before other pills) usually solves the problem. This is a minor, well-known issue, not a dangerous one. Since over-the-counter thyroid extract is unpredictable anyway, please go over your full routine with your pharmacist or doctor.
