Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here's the thing about this pairing: the interaction on file is really about how oral thyroid medicine gets absorbed from your stomach, when other foods or pills block it. But urokinase isn't a pill you swallow. It's a clot-busting drug given straight into a vein in the hospital, usually for serious blood clots, so the whole "empty stomach, timing, absorption" idea doesn't really apply to it.

Because urokinase skips the digestive tract entirely, thyroid extract shouldn't meaningfully change how it works. That said, urokinase carries a real bleeding risk on its own, so this isn't a situation for self-treating. Please let the medical team managing your treatment know about any supplement you take, including thyroid extract.
