CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Hydrocodone, Ibuprofen falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Tinospora Cordifolia interacts with.
In vitro research shows that Tinospora cordifolia extract inhibits CYP2D6 at high concentrations. However, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing about Vicoprofen (which combines hydrocodone with ibuprofen): the hydrocodone part is actually a prodrug. Your body uses an enzyme called CYP2D6 to turn it into hydromorphone, the form that does most of the pain-relieving work. Lab tests suggest Tinospora cordifolia (Giloy) might block CYP2D6, at least in a test tube at high concentrations.
If that blocking happened in real life, you'd likely get less conversion to the active form, so your pain relief could feel weaker. The honest part: this has only been seen in lab studies, never reported in people, so it's theoretical. Still, if your pain medicine isn't working as expected after starting Giloy, mention it to your pharmacist or doctor before changing anything.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

In vitro research shows that Tinospora cordifolia extract inhibits CYP2C9. Animal research shows that Tinospora cordifolia extract 400 mg/kg twice daily for 14 days reduces the clearance and increases plasma levels of glyburide, a CYP2C9 substrate. However, this interaction has not been reported in humans.

Vicoprofen pairs two medicines: hydrocodone (an opioid for pain) and ibuprofen (an NSAID). The ibuprofen half is partly broken down by a liver enzyme called CYP2C9. In test tube and animal studies, Giloy (Tinospora cordifolia) seems to slow that enzyme down, which could let ibuprofen build up a bit more than usual.
If that happened, you might be more likely to see typical ibuprofen side effects, like stomach upset, heartburn, or, with heavier use, stomach irritation or kidney strain. I want to be honest, though: this is only theoretical and has not been shown in people, so I don't want to overstate it. There have also been reports of liver problems with Giloy itself. Please check with your pharmacist or doctor before combining them.
