Here's the simple version: white mulberry leaf can gently lower blood sugar on its own, mostly by slowing how fast you absorb sugar after a meal. Your Metformin XR is already working to keep your numbers down too. Stack the two together and you could nudge your blood sugar lower than expected, which means a bit more chance of a low.

Honestly, this is more of a theory based on how each one works than something proven in big studies, so I wouldn't panic. A little mulberry tea now and then is unlikely to cause trouble. Concentrated capsules are where I'd be more careful. If you notice shakiness, sweating, dizziness, or feeling foggy, that can be a low. Check with your pharmacist or doctor before adding it, and keep an eye on your glucose readings.

Theoretically, white mulberry leaf might slow the elimination and increase the adverse effects of drugs which are OCT2 substrates.
Animal research shows that coadministration of white mulberry leaf extract with metformin, an OCT2 substrate, slows the renal elimination of metformin via inhibition of OCT2 activity. OCT2 is expressed in the kidneys and is responsible for transporting cationic drugs into tubular epithelial cells in order to be excreted in the urine.

Here's the gist for your Metformin XR: your kidneys clear this drug out of your body using a little 'shuttle' called OCT2. White mulberry leaf may slow that shuttle down, which in theory could let metformin hang around a bit longer than usual. More metformin sticking around can mean more of its typical effects, including upset stomach, nausea, loose stools, or a stronger drop in blood sugar (feeling shaky, sweaty, or lightheaded), especially since mulberry itself can lower blood sugar after meals.

That said, I want to be honest about the evidence here: this comes from animal research, not human studies, so it's more of a theoretical caution than a proven problem. Tea or food amounts of mulberry are unlikely to be an issue, but concentrated supplements are worth running by your pharmacist or doctor first.
