Clinical research shows that white mulberry leaf can lower blood glucose levels. Advise patients to monitor glucose levels closely.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: both metformin and white mulberry leaf can nudge your blood sugar downward, just by different paths. Metformin lowers the sugar your liver makes and helps your body use insulin better. White mulberry leaf contains a compound (DNJ) that slows how fast you absorb sugar from a meal. Stack the two and, in theory, your numbers could dip lower than expected.
This isn't a dangerous chemical clash, and it's based mostly on how these things work rather than strong human studies. Still, watch for low blood sugar signs like shakiness, sweating, dizziness, or feeling foggy, especially around meals. Food or tea amounts are unlikely to cause trouble, but concentrated supplements deserve a quick chat with your pharmacist or doctor first, and keep checking your glucose.

ORGANIC CATION TRANSPORTER 2 (OCT2) SUBSTRATES

Animal research shows that coadministration of white mulberry leaf extract with metformin, an OCT2 substrate, slows the renal elimination of metformin via inhibition of OCT2 activity. OCT2 is expressed in the kidneys and is responsible for transporting cationic drugs into tubular epithelial cells in order to be excreted in the urine.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for you: your kidneys clear metformin out of your body using a little 'shuttle' called OCT2. White mulberry leaf may slow that shuttle down, at least based on early animal studies. If less metformin gets flushed out, more can hang around in your system, which could nudge up the usual side effects, things like an upset stomach, nausea, loose stools, or a metallic taste.
Now, the honest part: this is theoretical and comes from lab and animal work, not solid human studies, so I wouldn't lose sleep over it. A bit of mulberry tea is very different from a concentrated supplement. Since both mulberry and metformin can lower blood sugar, the bigger thing to watch is feeling shaky or lightheaded. Please check with your pharmacist or doctor before adding a mulberry supplement.
