Here's the simple version: your Janumet already lowers blood sugar through two medicines, metformin and sitagliptin. White mulberry leaf can do a little of that work too, mainly by slowing how fast sugar from your meal gets absorbed. Stack the two together and your blood sugar could dip lower than expected.

Honestly, this concern is more theoretical than proven, based on how mulberry behaves rather than strong human studies. A cup of mulberry tea now and then is unlikely to cause trouble, but concentrated capsules are a different story. If you do try them, watch for low blood sugar signs like shakiness, sweating, dizziness, or feeling foggy, and loop in your pharmacist or doctor before starting.

Theoretically, white mulberry leaf might slow the elimination and increase the adverse effects of drugs which are OCT2 substrates.
Animal research shows that coadministration of white mulberry leaf extract with metformin, an OCT2 substrate, slows the renal elimination of metformin via inhibition of OCT2 activity. OCT2 is expressed in the kidneys and is responsible for transporting cationic drugs into tubular epithelial cells in order to be excreted in the urine.

Here's the gist with Janumet, which combines metformin and sitagliptin. Your kidneys use a little transporter called OCT2 to push metformin out into your urine. In animal studies, white mulberry leaf seemed to slow that transporter down, meaning metformin could clear out a bit more slowly and build up. If that happens, you might notice more of metformin's usual stomach complaints like nausea, an upset belly, or loose stools.

I want to be honest though: this is theoretical and based only on animal data, not people, so it's not something to panic over. On top of that, mulberry itself can lower blood sugar, so stacking it with Janumet could nudge your numbers lower than expected. If you're thinking about a concentrated mulberry supplement, just run it by your pharmacist or doctor first.
