The evidence for these mushrooms is limited in the data we hold. Turkey tail is rated Possibly Effective for cancer, but the others—lion's mane, reishi, cordyceps, and chaga—have insufficient evidence for their commonly marketed uses, including cognitive function, athletic performance, blood sugar control, and cardiovascular health.
That doesn't mean they don't work; it means the clinical studies we have aren't robust enough yet to draw a firm conclusion.
How safe is it?
Well-documented data
All five mushrooms are generally well tolerated in the short term, but long-term safety hasn't been thoroughly studied. The most common side effects are gastrointestinal: nausea, stomach discomfort, diarrhea, and constipation.
Lion's mane and reishi may also cause skin rash or dizziness. Chaga carries a specific caution: excessive use has been linked to kidney problems due to its high oxalate content.
There is also one case report of liver inflammation possibly tied to cordyceps, though it's unclear whether cordyceps or a contaminant was responsible. During pregnancy and breastfeeding, there isn't enough safety data for any of these ingredients—the facts advise against their use.

The most serious interaction we've documented is with blood thinners (anticoagulants and antiplatelet drugs) — lion's mane, reishi, cordyceps, and chaga all may increase bleeding risk at Moderate severity, and the effect is theoretically most concerning with chaga and cordyceps, which inhibit platelet function in lab studies.

All five ingredients interact with blood sugar medications (antidiabetes drugs) at Moderate severity, potentially lowering blood sugar further. Reishi and lion's mane also interact with blood pressure medications (antihypertensives), carrying a Moderate risk of excessive blood pressure drop.
Three ingredients—lion's mane, cordyceps, and chaga—may interfere with immune-suppressing medications used after transplants or for autoimmune conditions.
Turkey tail mushroom has two additional concerns: it may slow how your body absorbs tamoxifen (a breast cancer medication) and may increase blood levels of cyclophosphamide (a chemotherapy drug), both at Moderate severity. It also theoretically inhibits an enzyme (CYP2C9) that breaks down certain blood thinners and pain relievers, a Minor interaction.

Anticoagulant/Antiplatelet Drugs
Theoretically, high doses of reishi mushroom might increase the risk of bleeding.
A dose of 1.5 grams daily of reishi mushroom does not seem to decrease platelet aggregation, but a higher dose of 3 grams daily does.
Antidiabetes Drugs
Theoretically, reishi mushroom might have additive effects with antidiabetes drugs.
Animal research suggests that reishi mushroom decreases blood sugar. However, in patients with type 2 diabetes, taking reishi mushroom does not reduce fasting glucose levels, and its effects on glycated hemoglobin are inconsistent.
Antihypertensive Drugs
Theoretically, concurrent use of reishi mushroom with antihypertensive drugs might increase the risk of hypotension.
Reishi mushroom has shown hypotensive activity in animal research. Clinical evidence suggests that reishi mushroom reduces blood pressure in some, but not all, patients with hypertension.

Animal research suggests that an aqueous extract of lion's mane mushroom can reduce serum glucose and increase serum insulin.
Immunosuppressants
Theoretically, concurrent use of lion's mane mushroom might interfere with immunosuppressive therapy.
In animal and in vitro research, lion's mane mushroom polysaccharides stimulate the immune system.

Anticoagulant/Antiplatelet Drugs
Theoretically, chaga may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In vitro and animal research suggests that chaga extract can inhibit platelet aggregation. This effect has not been reported in humans.
Antidiabetes Drugs
Theoretically, taking chaga with antidiabetes drugs might increase the risk of hypoglycemia.

organic Cordyceps Mushroom extract
Anticoagulant/Antiplatelet Drugs
Theoretically, cordyceps may increase the risk of bleeding when used with antiplatelet or anticoagulant drugs.
In vitro and animal research suggests that cordyceps extract inhibits platelet aggregation and function. However, this interaction has not been reported in humans.
Immunosuppressants
Theoretically, concurrent use of cordyceps might interfere with immunosuppressive therapy.
Animal and in vitro research suggests that cordyceps stimulates the immune system. However, limited clinical research suggests that taking cordyceps may lower the necessary therapeutic dose of the immunosuppressant cyclosporine, which suggests that cordyceps may have an immunosuppressive effect.
Testosterone
Theoretically, concurrent use of cordyceps and testosterone might have additive effects.
Animal research suggests that cordyceps can increase testosterone levels. The clinical significance of this finding is unclear.
Turkey Tail Mushroom Extract
Antidiabetes Drugs
Theoretically, taking turkey tail mushroom with antidiabetes drugs might increase the risk of hypoglycemia.
Animal research suggests that turkey tail mushroom and a polysaccharide component can have hypoglycemic effects.
Cyclophosphamide
Theoretically, the polysaccharide peptide (PSP) component of turkey tail mushroom might increase exposure to cyclophosphamide.
Some animal research shows that the PSP component of turkey tail mushroom can increase the area under the concentration-time curve (AUC) of cyclophosphamide by 44% to 50% and the half-life by 34% to 43%. This interaction could potentially increase the effects and adverse effects of cyclophosphamide. However, it is not known whether PSP affects the levels of the active metabolites of cyclophosphamide that are responsible for its clinical activity.
Tamoxifen (Nolvadex)
Theoretically, the polysaccharide peptide (PSP) component of turkey tail mushroom might interfere with the absorption of tamoxifen.
Animal research suggests that PSP increases the time to reach maximum concentration of a single dose of tamoxifen by about 9.5 hours, or 228%. When repeated doses of tamoxifen were given, the time to reach maximum concentration was increased by about 5.6 hours, or 93%. However, PSP did not affect the maximum concentration or the area under the curve of tamoxifen.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, the polysaccharide peptide (PSP) component of turkey tail mushroom might inhibit CYP2C9.
Laboratory research suggests that the PSP component of turkey tail mushroom dose-dependently inhibits CYP2C9. Theoretically, taking PSP with drugs metabolized by CYP2C9 might increase drug levels and the risk of adverse effects. However, this has not been reported in humans.
