Orally, agaricus mushroom is generally well tolerated when used as a food. The most common side effects from supplements are mild: abdominal discomfort, diarrhea, and nausea.

Rare but serious adverse effects include allergic reactions, liver damage (hepatotoxicity), and a lung condition called interstitial lung disease. A handful of cases of severe liver damage have been reported in women with cancer taking chemotherapy who also took agaricus supplements — the liver injury developed rapidly in some of those cases.

Allergic skin reactions have also been reported with certain agaricus products. Because supplement safety and long-term effects aren't well established, agaricus mushroom extract should be avoided during pregnancy and breastfeeding.
