Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains one active ingredient: bilberry fruit extract at 60 mg per capsule. Bilberry is a dark purple berry related to blueberries, and the extract concentrates its natural compounds — mainly anthocyanidins — into supplement form.
The capsule itself is made from vegetable cellulose, and the other ingredients (cellulose, dicalcium phosphate) are inactive fillers that help form and stabilize the supplement.
Does it work?
Not established
The evidence for bilberry extract is mixed and limited. For night vision, the data suggests it's probably ineffective — don't expect it to sharpen your eyesight.
For age-related cognitive decline, age-related macular degeneration (AMD), atherosclerosis, high cholesterol (hyperlipidemia), and chronic venous insufficiency (a condition affecting blood flow in leg veins), there simply isn't enough reliable evidence to say whether it works or not. If you're considering bilberry for any of these, talk it through with your doctor about whether there's a better-studied option.
How safe is it?
Well-documented data
Bilberry fruit eaten as food is generally well tolerated, but the safety data for concentrated supplements like this one is limited, especially over the long term. Most people who take it experience only minor side effects — dark-colored or greenish stools, flatulence, and mild stomach discomfort are the most common.
A small number of trial participants reported mild to moderate gas, and temporary GI upset has been noted in some studies. Your tongue and teeth may also temporarily stain dark blue or black.
Because long-term safety isn't well established, the caution is to stick with food amounts of bilberry rather than supplement doses if you can.
Meds to double-check
Moderate interaction found
Before you take this product, check with your doctor or pharmacist if you're on blood thinners or antiplatelet drugs (Moderate risk), diabetes medications (Moderate risk), drugs metabolized by the CYP2E1 enzyme, or the cancer drug erlotinib (Tarceva) — all are Moderate severity. These interactions haven't been proven in humans, but the theoretical risk is real enough to deserve a conversation.
The bottom line
Scorecard at a glanceFully disclosed formula with no established evidence rating for its marketed use. Moderate medication interactions have been identified, and safety information is well characterized.
Bilberry extract is generally well tolerated in the short term, but the evidence it actually works for vision, cognition, or heart health is weak or absent. If you take blood thinners, diabetes medications, or certain cancer drugs, this supplement could cause problems — check with your doctor or pharmacist first.
For most other people, the main risk is an upset stomach and dark stools, but limited long-term safety data means you should talk it over with your own healthcare provider before starting.

Bilberry Extract 60 mg by GNC Herbal Plus Drug Interactions
HelloPharmacist Interaction Report
GNC Herbal Plus Bilberry Extract 60 mg contains bilberry fruit extract, which has documented interactions with several medication types.
The most serious concern is with blood thinners and antiplatelet drugs (like warfarin, aspirin, and clopidogrel) — research suggests bilberry may increase your bleeding risk by making platelets less sticky. If you take any of these, check with your doctor or pharmacist before starting.
Read the full breakdown — every affected drug type, severity by severity
Bilberry also interacts with diabetes medications. It may lower your blood sugar on its own, so combining it with antidiabetes drugs could theoretically push your glucose too low.
Additionally, bilberry may speed up how your liver breaks down certain drugs metabolized by the CYP2E1 enzyme pathway, potentially making them less effective — and it may reduce how well the cancer drug erlotinib (Tarceva) works.
Additionally, run your exact medications through the interaction checker on this page. Altogether, these interactions span 275 individual medications.

For each ingredient in Bilberry Extract 60 mg with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
Bilberry fruit extract
Anticoagulant/Antiplatelet Drugs
Theoretically, bilberry fruit extract might increase the risk of bleeding when taken with anticoagulant or antiplatelet drugs.
In vitro, animal, and clinical research suggest that anthocyanidin extracts from bilberry can inhibit platelet aggregation.
Antidiabetes Drugs
Theoretically, bilberry leaf or fruit extract may increase the risk of hypoglycemia when taken with antidiabetes drugs.
Animal research suggests that bilberry leaf extract might have blood glucose-lowering activity. Also, one small clinical trial in patients with type 2 diabetes shows that taking bilberry fruit extract 470 mg as a single dose prior to an oral glucose tolerance test lowers plasma glucose levels when compared with placebo.
Cytochrome P450 2E1 (Cyp2E1) Substrates
Theoretically, bilberry fruit extract might decrease levels of drugs metabolized by CYP2E1.
Animal research shows that exposure to small concentrations of bilberry extract in drinking water for around one month increased CYP2E1 activity by 31%. However, exposure over a 2-month period did not increase CYP2E1 activity. This effect has not been reported in humans.
Erlotinib (Tarceva)
Theoretically, bilberry fruit extract might reduce the efficacy of erlotinib.
In vitro research suggests that bilberry fruit extract and its constituents, delphinidin and delphinidin-3-O-glucoside, inhibit the activity of erlotinib. This interaction has not been reported in humans.
