What’s inside
Low disclosure
EnzymeMD contains 19 ingredients, most of them digestive enzymes. The active ones include papain and bromelain (both protein-digesting enzymes from plants), along with lipase (breaks down fat), lactase (breaks down milk sugar), and several other carbohydrate- and protein-splitting enzymes.
The blend also includes Saccharomyces boulardii, a beneficial yeast. Several ingredients—Cellulase, Peptidase, Invertase, Glucoamylase, Alpha-Galactosidase, Xylanase, Maltase, Beta-Glucanase, and Hemicellulase—are also enzyme-based digestive aids.
The inactive ingredients are rice flour, vegetable cellulose capsule, and silica.
Does it work?
Strong evidence
The evidence for most ingredients in this product is not established. Lactase is effective for lactose intolerance—that's the clearest rating we have.
Saccharomyces boulardii is possibly effective for rotaviral diarrhea, travelers' diarrhea, antibiotic-associated diarrhea, necrotizing enterocolitis (a serious intestinal inflammation in premature infants), Clostridioides difficile infection, and Helicobacter pylori. For papain, bromelain, lipase, phytase, and the various proteases, the evidence is insufficient to rate their effectiveness for the conditions studied.
We don't have effectiveness data on file for several of the other enzymes in the blend.
How safe is it?
Well-documented data
Most of the active enzymes in this product are generally well tolerated at appropriate doses, though high oral doses of papain may not be safe. The most common side effects are digestive—diarrhea, flatulence, gastric upset, headache—and allergic reactions in people sensitive to any of the enzymes.
Papain in particular can cause itchy watery eyes, runny nose, sneezing, abdominal cramps, sweating, and diarrhea in sensitive individuals; it may also irritate the digestive tract. Bromelain can cause gastrointestinal disturbances, including nausea, vomiting, and abdominal pain.
Saccharomyces boulardii is generally safe for healthy adults but should be avoided if you're critically ill or immunocompromised—there are rare reports of fungemia (yeast in the bloodstream) in critically ill patients, especially those with central lines or on broad-spectrum antibiotics. All three proteases in the blend are generally well tolerated but may cause digestive upset or allergic reactions.

EnzymeMD by 1MD Nutrition interacts with medications through its papain and bromelain content.
The most serious concern is papain with warfarin (Coumadin), a blood thinner — papain may increase warfarin's effects and side effects, and one case reported a patient's blood-thinning level (INR) spiking to 7.4 after taking a papain supplement while on stable warfarin therapy.

Bromelain raises the risk of bleeding when combined with blood thinners and antiplatelet drugs (such as aspirin, clopidogrel, and warfarin). There's one case report of minor bruising in someone taking bromelain with naproxen, a nonsteroidal anti-inflammatory.
Bromelain may also theoretically increase levels of tetracycline antibiotics, though one study in healthy adults found no difference in tetracycline blood levels when combined with bromelain.
Saccharomyces boulardii, a live yeast in this product, may be rendered less effective if you're taking antifungal medications simultaneously, since antifungals can kill the yeast organism.

Bromelain
Anticoagulant/Antiplatelet Drugs
Bromelain may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
There is one case report of a patient experiencing minor bruising while taking bromelain with naproxen. Bromelain is thought to have antiplatelet activity. Whether this interaction is of concern with topical bromelain is unclear. Interference with coagulation of burn wounds has been reported in a patient receiving bromelain-based enzymatic debridement. However, observational research has found that topical bromelain debridement is not associated with increases or decreases in laboratory markers of coagulation when compared with surgical debridement.
Tetracycline Antibiotics
Theoretically, bromelain might increase levels of tetracycline antibiotics.
Laboratory research suggests that bromelain might increase the absorption of tetracycline antibiotics. However, a study in healthy adults reported no difference in tetracycline plasma levels when a 500 mg dose was taken with or without bromelain 80 mg.
Saccharomyces boulardii
Antifungals
Theoretically, taking antifungals with Saccharomyces boulardii might decrease the effectiveness of Saccharomyces boulardii.
S. boulardii is a live yeast. Therefore, simultaneously taking antifungals might kill a significant number of the organisms.
Papain
Warfarin (Coumadin)
Theoretically, papain might increase the effects and side effects of warfarin.
In one case report, a patient previously stable on warfarin was found to have an international normalization ratio (INR) of 7.4, which was attributed to ingestion of a supplement containing papain from papaya extract.
