Flora Extreme contains 15 active ingredients — a mix of probiotic bacteria and prebiotic fibers designed to support digestive and gut health. The bacterial strains include several Lactobacillus species (salivarius, paracasei, casei, bulgaricus, acidophilus, and plantarum), multiple Bifidobacterium strains (longum, bifidum, breve, and lactis), and two Bacillus species (coagulans and subtilis).
The product also includes a Prebiotic Fiber Blend and a Probiotic Daily Blend (which contain fiber from gum arabic, chicory root powder, and Jerusalem artichoke) to feed the beneficial bacteria. Inactive ingredients are present as excipients: hypromellose, microcrystalline cellulose, magnesium stearate, and silica dioxide help form and stabilize the capsule.

The evidence for Flora Extreme's ingredients is mixed. Lactobacillus acidophilus shows possibly effective evidence for irritable bowel syndrome, Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis — though it may be ineffective for atopic disease.
Bacillus coagulans is possibly effective for constipation and irritable bowel syndrome. Bifidobacterium bifidum is possibly effective for irritable bowel syndrome and respiratory tract infections.
Bacillus subtilis is possibly effective for antibiotic-associated diarrhea. For most other conditions the product's ingredients claim to address — including allergies, asthma, cognitive decline, anxiety, athletic performance, and various other conditions — the evidence is insufficient to establish whether they work.
Chicory and gum arabic have not been reliably studied for the conditions listed in the product facts.
How safe is it?
Well-documented data
The probiotic bacteria in Flora Extreme are generally well tolerated in healthy people, but caution is needed if you're very ill, immunocompromised, or in the hospital with a central line. Rare cases of serious bacterial infection (bacteremia and sepsis) have occurred in critically ill patients taking similar probiotics.
Gastrointestinal side effects — bloating, flatulence, abdominal discomfort, and diarrhea — have been reported rarely with individual strains; one person reported a dry cough. Gum arabic can cause digestive upset including bloating, flatulence, nausea, vomiting, and mild diarrhea; these often subside within 2 weeks.
Chicory commonly causes gas and bloating and can trigger allergies in people sensitive to the ragweed family. Chicory is possibly unsafe in pregnancy (some sources warn it may stimulate the uterus) and lacks reliable safety data while breastfeeding.
The pregnancy and lactation safety profile for most of the bacterial strains is not clearly established in the data we hold.

A small study in healthy volunteers shows that taking amoxicillin and gum arabic concurrently significantly reduces the absorption of amoxicillin. Separate doses of amoxicillin from gum arabic by at least 2 hours.
Oral Drugs
Theoretically, gum arabic can alter the absorption of oral drugs due to its fiber content.
Gum arabic has been used as a suspending osmotic agent in drug formulations. It might improve bioavailability of water-insoluble drugs like naproxen, but reduce absorption of polar drugs like amoxicillin. To avoid changes in absorption, take gum arabic 30-60 minutes after oral medications.

Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
Chicory root powder
Antidiabetes Drugs
Theoretically, chicory might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Animal and in vitro research shows that chicory extracts have antidiabetic effects.
