The evidence supporting either ingredient is insufficient to establish how well they work. Horny Goat Weed Aerial Part Extract has been studied for sexual dysfunction, osteoporosis, coronary heart disease, and bronchitis, but the data we hold shows insufficient reliable evidence for any of these uses.
Maca Root Extract has similarly been studied for multiple conditions — including sexual dysfunction, athletic performance, osteoporosis, and anemia — but again, the evidence is insufficient to rate its effectiveness. How safe is it?
Well-documented data
Horny Goat Weed is generally well tolerated when used short-term, though human safety data is limited and product quality varies. The most common side effects are dizziness, dry mouth, nosebleed, thirst, and vomiting.
Long-term use carries similar risks, and a rare serious effect — respiratory arrest — has been reported. One case of an abnormal heart rhythm (tachyarrhythmia) was reported in a 66-year-old with heart disease after 2 weeks of use, though it's unclear if horny goat weed alone caused it.
Maca Root Extract is generally well tolerated as a food but hasn't been studied long-term as a supplement; fresh, uncooked maca may cause stomach pain. Safety data during pregnancy is insufficient for both ingredients — the facts advise against horny goat weed in pregnancy and note not enough data exists for maca.
For breastfeeding, there isn't enough reliable information on either ingredient. Talk with your doctor or pharmacist about whether this product is right for you, especially if you're pregnant or nursing.

Additionally, the extract inhibits several liver enzymes (CYP1A2, CYP2B6, and CYP3A4), which could increase the effects and side effects of drugs your liver normally breaks down through these pathways — again, this is theoretical in humans but well documented in the lab. Lastly, if you take estrogen-containing medications or hormone therapy, horny goat weed's estrogenic activity may amplify both therapeutic and unwanted effects.

Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, horny goat weed might increase the effects and side effects of CYP1A2 substrates.
In vitro, horny goat weed leaf extract inhibits CYP1A2. This effect has not been reported in humans.
Cytochrome P450 2B6 (Cyp2B6) Substrates
Theoretically, horny goat weed might increase the effects and side effects of CYP2B6 substrates.
In vitro, horny goat weed leaf extract inhibits CYP2B6. This effect has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.
Estrogens
Theoretically, concomitant use of horny goat weed with estrogens might increase their therapeutic and adverse effects.
In vitro evidence suggests that horny goat weed has estrogenic activity. In clinical research, horny goat weed has been shown to increase blood levels of estrogen in some females.
