Amitriptyline falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP1A2 substrates.
In vitro, horny goat weed leaf extract inhibits CYP1A2. This effect has not been reported in humans.

Your body uses several liver enzymes to clear amitriptyline, and one of them is called CYP1A2. In test-tube research, horny goat weed appears to slow that enzyme down. If that same effect happened in people, your amitriptyline could break down a little more slowly and build up, which might make its effects and side effects stronger. With this medicine that could mean more drowsiness, dry mouth, constipation, dizziness, or a faster heartbeat.

Here's the reassuring part: this is only a theoretical concern seen in lab studies, not something reported in actual patients. Still, since amitriptyline already has plenty of side effects, it's worth a quick chat with your pharmacist or doctor before combining the two.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Here's the gist: your body partly relies on a liver enzyme called CYP3A4 to clear amitriptyline. In test-tube studies, horny goat weed seems to slow that enzyme down. If the same thing happened in people, amitriptyline could clear more slowly and build up a bit, which might nudge up its usual side effects like drowsiness, dry mouth, constipation, blurry vision, dizziness on standing, or a faster heartbeat.

The reassuring part: this has only been seen in lab dishes, not in actual patients, so it's theoretical. Amitriptyline is not a prodrug, so the direction here is more drug effect, not less. Still, I'd run it past your pharmacist or doctor before combining them, especially if your dose is on the higher side.
