Prickly pear cactus is possibly effective for diabetes according to the data we hold. For other uses — age-related cognitive decline, benign prostatic hyperplasia (enlarged prostate), indigestion (dyspepsia), and high cholesterol — there isn't enough reliable evidence in our data to say whether it works.
How safe is it?
Well-documented data
Prickly pear cactus is generally well tolerated when taken by mouth. The most common side effects are mild and digestive: abdominal fullness, indigestion, mild diarrhea, and nausea.
Rare cases of headache, dizziness, and insomnia have been reported in clinical studies. Very rarely, the dried cactus material can swell and cause serious blockages, but this is extremely uncommon and linked to eating a large number of cactus fruits.
During pregnancy, there isn't enough safety data to support using supplement amounts of prickly pear cactus — talk with your doctor or pharmacist about whether it's right for you. The same applies to breastfeeding.

Combining prickly pear cactus with antidiabetes drugs might increase the risk of hypoglycemia.
Case reports show that combining prickly pear cactus with antidiabetes drugs such as chlorpropamide, glyburide, glipizide, and metformin can increase the risk of hypoglycemia in patients with type 2 diabetes. Advise patients to monitor glucose levels closely. Dose adjustments may be necessary.
