Peppermint oil is likely effective for irritable bowel syndrome (IBS). It's possibly effective for dyspepsia (indigestion), chemotherapy-induced nausea and vomiting, barium enema-related colon cramping, and adverse effects from endoscopy procedures.

How safe is it?
Well-documented data
Peppermint oil in softgel form is generally well tolerated when taken by mouth. The most common side effects are abdominal pain, anal burning, belching, diarrhea, dry mouth, heartburn, nausea, and vomiting.
Allergic reactions are rare but have been reported, including one case of anaphylaxis (severe swelling of the lips, tongue, and throat) after peppermint candy. Because this is a concentrated oil supplement, it should be used only as directed and not taken without your provider's approval, especially if you have a history of asthma or allergies.

The most serious concern is with cyclosporine (Neoral, Sandimmune), an immunosuppressant: peppermint oil may slow how your body clears it, potentially raising its levels and side effects. Animal research suggests peppermint oil inhibits the enzyme (CYP3A4) that breaks down cyclosporine, though this hasn't been confirmed in humans yet.

Peppermint oil may also affect three major enzyme systems your liver uses to metabolize drugs. In lab and animal studies, it inhibits CYP3A4, CYP2C9, and CYP2C19—meaning it could theoretically increase blood levels of medications that depend on these enzymes to be cleared.
For CYP3A4, there's clinical evidence: one dose of 600 mg peppermint oil inhibited the enzyme and raised levels of felodipine, a heart medication. For CYP2C9 and CYP2C19, the data is from test-tube and animal work so far, with no human cases reported yet.
Peppermint oil may also theoretically increase levels of drugs that use the CYP1A2 enzyme, though in a clinical trial, peppermint tea did not noticeably affect caffeine metabolism. Altogether, these interactions span 797 individual medications.

Peppermint oil
Cyclosporine (Neoral, Sandimmune)
Theoretically, peppermint oil might increase the levels and adverse effects of cyclosporine.
In animal research, peppermint oil inhibits cyclosporine metabolism and increases cyclosporine levels. Inhibition of cytochrome P450 3A4 (CYP3A4) may be partially responsible for this interaction. An interaction between peppermint oil and cyclosporine has not been reported in humans.
Cytochrome P450 2C19 (Cyp2C19) Substrates
Theoretically, peppermint might increase the levels of CYP2C19 substrates.
In vitro research shows that peppermint oil inhibits CYP2C19. So far, this interaction has not been reported in humans.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, peppermint might increase the levels of CYP2C9 substrates.
In vitro research shows that peppermint oil inhibits CYP2C9. So far, this interaction has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, peppermint might increase the levels of CYP3A4 substrates.
Clinical research in healthy volunteers shows that a single dose of peppermint oil 600 mg inhibits CYP3A4 enzymes and increases the AUC of felodipine, a CYP3A4 substrate. However, in vitro research suggests that peppermint oil only inhibits CYP3A4 at very high concentrations.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, peppermint might increase the levels of CYP1A2 substrates.
In vitro and animal research shows that peppermint oil and peppermint leaf inhibit CYP1A2. However, in clinical research, peppermint tea did not significantly affect the metabolism of caffeine, a CYP1A2 substrate. It is possible that the 6-day duration of treatment may have been too short to identify a difference.
