Does it work?
Moderate evidence
The evidence for these strains is mixed. Lactobacillus acidophilus and Bifidobacterium bifidum are possibly effective for irritable bowel syndrome (IBS), and Lactobacillus acidophilus also shows possible effectiveness for antibiotic-associated diarrhea, Helicobacter pylori infection, and bacterial vaginosis.
Lactobacillus sporogenes is possibly effective for constipation and IBS. Saccharomyces boulardii is possibly effective for several types of diarrhea—rotaviral, travelers', antibiotic-associated—and also for Clostridioides difficile infection and Helicobacter pylori.
For most other conditions listed, the evidence is insufficient to rate these strains reliably. If you're taking this for a specific health concern, talk it over with your doctor to see if one of these strains has decent evidence behind it.
How safe is it?
Well-documented data
These probiotics are generally well tolerated in healthy adults and children. However, caution is warranted if you're seriously ill, immune-compromised, have indwelling catheters, or are a preterm infant—rare but serious infections (bacteremia and sepsis) have been reported in these populations.
Common mild side effects are rare but can include abdominal discomfort, bloating, flatulence, and constipation. One case of dry cough was reported.
Since the organisms are live, there's a theoretical risk of infection in vulnerable patients. For pregnancy and lactation, Bifidobacterium longum, Lactobacillus acidophilus, Bifidobacterium bifidum, and Streptococcus thermophilus are rated possibly safe, but data are limited—check with your doctor before use.
Pregnancy and lactation data aren't on file for Lactobacillus sporogenes, Lactobacillus gasseri, or Saccharomyces boulardii.

Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
