ProbioMax Complete DF contains 18 active ingredients—all live probiotic bacteria strains from the Lactobacillus, Bifidobacterium, Streptococcus, and Lactococcus families. The checked strains with documented uses are Lactobacillus acidophilus (possibly effective for IBS, H. pylori, antibiotic-associated diarrhea, and bacterial vaginosis), Bifidobacterium bifidum (possibly effective for IBS and respiratory tract infections), and Streptococcus thermophilus (evidence not yet established for the conditions we tracked).
The formula also includes inactive ingredients like potato maltodextrin, hypromellose, gellan gum, and magnesium stearate as fillers and capsule materials.
Does it work?
Moderate evidence
The evidence we hold is mixed and sparse. Lactobacillus acidophilus is rated possibly effective for IBS, H. pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis—but possibly ineffective for atopic disease.
Bifidobacterium bifidum is possibly effective for IBS and respiratory tract infections. For most of the other strains and most other conditions studied, the evidence is insufficient to say either way.
Streptococcus thermophilus has insufficient data for any of the conditions we reviewed, including antibiotic-associated diarrhea, bacterial vaginosis, constipation, and diabetes. If you're taking this for a specific reason, ask your pharmacist whether the evidence supports it for your goal.
How safe is it?
Well-documented data
The checked ingredients are generally well tolerated in healthy adults and children. However, the data come with important caveats.
Since these are live organisms, there is a small theoretical risk of infection in patients who are very ill, have weakened immune systems, or have indwelling catheters—such cases are rare but have been reported. Mild gastrointestinal side effects like bloating, flatulence, abdominal discomfort, and diarrhea are uncommon but can occur when starting probiotics.
One clinical trial reported rare cases of rash and itching in a small number of participants, though it's unclear whether the probiotics or another ingredient caused them. If you're critically ill or immunocompromised, check with your doctor first.

Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
Howaru Bifidobacterium longum infantis BI-26
Antibiotic Drugs
Theoretically, taking Bifidobacterium breve with antibiotic drugs might decrease the effectiveness of B. breve.
Since B. breve preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. breve preparations by at least 2 hours.
Howaru Bifidobacterium longum BL-5
Antibiotic Drugs
Theoretically, taking Bifidobacterium longum with antibiotic drugs might decrease the effectiveness of B. longum.
