up4 Ultra is a 9-ingredient probiotic blend built on four Bifidobacterium strains and five Lactobacillus strains—different species of live, beneficial bacteria meant to support your gut. Each ingredient is labeled with its own strain designation so you know exactly what you're getting.
The capsule also contains inactive ingredients (cellulose, fructooligosaccharides, plant magnesium stearate, and silicon dioxide) that serve as binders and fillers.

The evidence behind these probiotics is mixed. Lactobacillus acidophilus is possibly effective for irritable bowel syndrome, Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis.
Bifidobacterium bifidum is possibly effective for IBS and respiratory tract infections. Bifidobacterium longum and Bifidobacterium breve have insufficient evidence or were rated ineffective for the conditions we have data on—meaning we don't yet know if they work for those purposes.

How safe is it?
Well-documented data
These probiotics are generally well tolerated in healthy adults and children. The most common minor side effects are gastrointestinal—bloating, flatulence, abdominal discomfort, or diarrhea—but these are uncommon.
One clinical trial reported rare cases of rash and itching with a different probiotic combination, though it's unclear whether the probiotics themselves caused it. The facts note a theoretical concern: because these are live organisms, they could theoretically cause infection in people with severely weakened immune systems or serious illness, though such cases are rare.
Pregnancy is rated possibly safe for both Lactobacillus acidophilus and Bifidobacterium species, though data are limited; lactation safety data aren't on file for most of these ingredients.
