Ibuprofen falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Aconite interacts with.

Theoretically, combining aconite with other antiplatelet or anticoagulant drugs might increase the risk of bruising and bleeding.
Higenamine, a constituent of aconite, is thought to have antiplatelet and antithrombotic effects. In an animal model of thrombosis, higenamine inhibited platelet aggregation and reduced the size of thrombus formation.

Here's the short version: ibuprofen already nudges your platelets to be a little less sticky, which is part of why it can cause stomach upset, easy bruising, or bleeding, especially with regular use. Aconite contains a compound called higenamine that, in lab and animal studies, also seemed to make platelets clump less and slowed clot formation. So in theory, stacking the two could add up and raise your chances of bruising or bleeding.

I want to be honest about the evidence though: this is based only on test-tube and animal work, not real patients, so it's more of a caution than a proven problem. The bigger concern is aconite itself, which is genuinely toxic and best avoided. Please don't change your ibuprofen, and talk with your pharmacist or doctor before using anything containing aconite.

Aconite is a highly poisonous plant, and even small amounts of the raw or improperly processed root can cause severe, life-threatening reactions. What Is Ibuprofen Used For?

Brand names: Motrin, Children's Motrin, Advil, Children's Advil, Fenbid Forte Gel, Ibugel Forte, Arthrofen, Ebufac, Rimafen, Calprofen, Fenpaed, Galprofen, Nurofen for Children, Orbifen for Children, Brufen, Brufen Retard, Fenbid, Tab-Profen, Ibuprofen, Calodor

Oral ibuprofen products (tablets, coated tablets, and liquid-filled capsules) are used to temporarily relieve minor aches and pains from headache, backache, toothache, muscular aches, menstrual cramps, minor arthritis pain, and the common cold, and to temporarily reduce fever. The oral prescription tablet (Ibu) is also indicated for the signs and symptoms of rheumatoid arthritis and osteoarthritis, mild to moderate pain, and primary dysmenorrhea (painful periods).

Caldolor, an intravenous (IV) form, is approved for adults and children 3 months and older to manage mild to moderate pain, to manage moderate to severe pain alongside opioid pain medicines, and to reduce fever. A separate intravenous solution, Ibuprofen Lysine, is used specifically in premature infants to close a patent ductus arteriosus (PDA) — an abnormal blood vessel connection near the heart — when standard medical management has not worked.
