Nelfinavir Mesylate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Aegeline interacts with.

Aegeline inhibits cytochrome P450 3A4 (CYP3A4) activity in vitro. So far, this interaction has not been reported in humans. Theoretically, aegeline may increase levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others. Use aegeline cautiously or avoid in patients taking these drugs.

Here's the short version: your HIV medicine nelfinavir is cleared by a liver enzyme called CYP3A4. In lab (test tube) studies, aegeline appears to slow that same enzyme down. On paper, that could let nelfinavir build up a bit in your body, which might mean more of its usual side effects like diarrhea, nausea, gas, or stomach upset. I want to stress this is theoretical, based only on test tube data, and it hasn't actually been seen in people.

Honestly, the bigger concern here is aegeline itself. It's been tied to serious liver injury and U.S. regulators have warned against it, so I'd steer clear regardless. Please don't change your nelfinavir, and check with your pharmacist or doctor before adding any supplement.
