Why this applies to Palovarotene

Palovarotene falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Aegeline interacts with.

Aegeline inhibits cytochrome P450 3A4 (CYP3A4) activity in vitro. So far, this interaction has not been reported in humans. Theoretically, aegeline may increase levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others. Use aegeline cautiously or avoid in patients taking these drugs.

Here's the short version for anyone taking Palovarotene (Sohonos). Your body clears this medicine using a liver enzyme called CYP3A4. In lab dish studies, aegeline appears to slow that same enzyme down. If that also happened in people, palovarotene could build up a bit higher than expected, which may mean stronger effects and a greater chance of its usual side effects like dry skin and lips, joint or muscle aches, headache, and skin peeling or rash.

I want to be honest about the evidence though. This is theoretical, based only on test-tube work, and it hasn't been reported in real patients. On top of that, aegeline has been tied to serious liver injury and regulators warned against it, so I'd steer clear regardless. Please run any supplement past your doctor or pharmacist first.

Aegeline is a compound originally identified in the bael tree (Aegle marmelos) that gained attention because it was added to some weight-loss and workout supplements. It is best known for being linked to serious liver injury in a product called OxyELITE Pro, and U.S. regulators warned against products containing it. There is no good evidence it safely helps with weight loss, and it should generally be avoided.

What Is Palovarotene Used For?

Brand name: Sohonos

Sohonos (palovarotene) is used to reduce the volume of new heterotopic ossification — abnormal bone that forms in muscles and soft tissue — in people with fibrodysplasia ossificans progressiva (FOP). It is approved for adults and for children aged 8 years and older (females) and 10 years and older (males).

FOP is a rare genetic condition where the body progressively forms extra bone outside the skeleton, especially after injury or flare-ups. Sohonos is taken both as a daily maintenance dose and at higher doses when a flare-up occurs.
