Palovarotene falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.

Here's the concern with pairing these two. Once your body turns adrafinil into modafinil, that modafinil can gently speed up a liver enzyme called CYP3A4. Palovarotene (Sohonos) is broken down by that same enzyme, so in theory a faster-working enzyme could clear it out of your system a little quicker and leave you with lower drug levels.

Why does that matter? Palovarotene is prescribed to slow the abnormal bone growth of FOP, and less drug in your body could mean it works less well for that. I'll be honest, this is a theoretical interaction based on how the drugs behave, not something proven in real patients, and modafinil is only a weak inducer. Still, since this is a serious, hard-to-treat condition, please loop in your doctor or pharmacist before combining them.
