Why this applies to Rolapitant Hydrochloride

Rolapitant Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Aegeline interacts with.

Aegeline inhibits cytochrome P450 3A4 (CYP3A4) activity in vitro. So far, this interaction has not been reported in humans. Theoretically, aegeline may increase levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others. Use aegeline cautiously or avoid in patients taking these drugs.

Here's the short version on combining aegeline with rolapitant (Varubi). Your body uses a liver enzyme called CYP3A4 to help clear rolapitant. In lab dish studies, aegeline appears to slow that enzyme down. If that happened in real life, rolapitant could build up a bit higher than usual, which might mean more of its usual side effects, things like hiccups, dizziness, decreased appetite, or tiredness.

I want to be honest about the evidence though: this is only seen in test tube research, not in actual patients, and it's rated as a minor concern. The bigger issue is aegeline itself, which has been tied to serious liver injury and is best avoided altogether. Please run any supplement use past your pharmacist or oncology team first.

Aegeline is a compound originally identified in the bael tree (Aegle marmelos) that gained attention because it was added to some weight-loss and workout supplements. It is best known for being linked to serious liver injury in a product called OxyELITE Pro, and U.S. regulators warned against products containing it. There is no good evidence it safely helps with weight loss, and it should generally be avoided.
