Rolapitant Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.

Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.

Here's the gist for someone using Varubi (rolapitant) to prevent the delayed nausea and vomiting that can hit a few days after chemo. Your liver clears rolapitant partly through an enzyme called CYP3A4. Adrafinil turns into modafinil in your body, and modafinil can mildly speed up that same enzyme. In theory, that could nudge rolapitant levels down a bit, which might make it a little less effective at keeping nausea and vomiting away.

I want to be honest about how soft this is: it's based on how these drugs behave on paper, not on real patient studies, so the actual impact is probably small. Still, since rolapitant is meant to protect you during a rough chemo stretch, it's worth a quick chat with your oncology pharmacist or doctor before mixing in adrafinil. Don't change anything on your own.
