Alogliptin falls under ANTIDIABETES DRUGS — the group Alfalfa interacts with.

Theoretically, alfalfa might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Animal research suggests that alfalfa decreases blood sugar in diabetic mice. Also, in one case report, a diabetic patient experienced hypoglycemia after consuming alfalfa extract. Monitor blood glucose levels closely. Dose adjustments might be necessary.

Here's the simple version: alfalfa may have a mild blood-sugar-lowering effect of its own. Since alogliptin (Nesina) is already working to bring your numbers down, stacking the two could, in theory, push your blood sugar lower than you want. That might show up as shakiness, sweating, dizziness, hunger, or feeling foggy.

I want to be honest about the evidence, though. This concern comes mainly from animal studies and a single patient report, so it's more of a theoretical caution than a proven problem. Food-sized amounts of alfalfa sprouts are unlikely to matter much; concentrated extract supplements are the bigger question mark. If you do try alfalfa, keep an eye on your glucose readings and loop in your pharmacist or doctor before changing anything.
