Ortho-Prefest combines estradiol (an estrogen) with norgestimate (a progestin) to ease menopause symptoms. Here's the thing about alfalfa: it carries plant compounds called coumestrol and isoflavonoids that act weakly like your body's own estrogen. In theory, layering these mild plant estrogens on top of your hormone therapy could nudge your overall estrogen-like activity, either subtly adding to it or competing with it.

I want to be honest, though. This concern is theoretical, based on how alfalfa behaves in the lab rather than real cases of problems in people. Small amounts in food are unlikely to matter, but concentrated supplements are a bigger question mark. Before adding alfalfa, just run it by your pharmacist or doctor first.
