Theoretically, atractylodes might increase the risk of bleeding when used concomitantly with anticoagulant and antiplatelet drugs.
Laboratory research suggests that atractylenolides II and III, constituents of atractylodes, reduce platelet activation. So far, this has not been shown in humans.

Here's the thing to know about Synalgos DC: it contains aspirin, which already thins the blood a bit by quieting your platelets (the cells that help you clot). Atractylodes may do something similar. In lab dishes, two of its compounds (atractylenolides II and III) made platelets less sticky. Stack that on top of aspirin, and in theory you could bruise more easily or bleed a little longer from a cut, a nosebleed, or your gums.

I want to be straight with you, though: this is only seen in lab and animal studies, never proven in people, so the real-world risk looks low. Still, since aspirin is in the mix, it's worth a quick chat with your pharmacist or doctor before combining them, especially if you take other blood thinners.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Here's the piece that matters in your Synalgos DC combo: the caffeine. Caffeine is broken down by a liver enzyme called CYP1A2. In animal studies using high doses, atractylodes seemed to speed that enzyme up, which could clear caffeine a little faster and make it feel weaker. In plain terms, your usual caffeine kick might not last as long. The aspirin and dihydrocodeine parts aren't really affected by this enzyme.

I want to be honest about how thin this evidence is. It's only from lab and animal work at high doses, and it has not been shown in people, so for most folks it's unlikely to cause real trouble. Still, if you plan to use atractylodes regularly, run it by your pharmacist or doctor first.

Theoretically, atractylodes might increase the levels of CYP3A4 substrates.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.

Here's the short version on combining atractylodes with Synalgos DC (aspirin, caffeine, and dihydrocodeine). The concern is that, in test tube and animal studies, high doses of atractylodes seemed to slow down a liver enzyme called CYP3A4. Dihydrocodeine is partly handled by that enzyme, so in theory blocking it could let a bit more of the drug linger in your system, which might mean more drowsiness, dizziness, constipation, or stomach upset.

That said, this has only been seen in lab and animal research, never confirmed in people, and the interaction is rated minor. I wouldn't lose sleep over it, but since this combo already affects breathing and alertness, please run it by your pharmacist or doctor before pairing them.
