Sirolimus (albumin bound) falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Atractylodes interacts with.

Theoretically, atractylodes might increase the levels of CYP3A4 substrates.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.

Here's the gist: your sirolimus (Fyarro) is cleared from your body by a liver enzyme called CYP3A4. In animal studies using high doses, atractylodes appeared to slow a similar enzyme down. If that same thing happened in people, your body might clear sirolimus more slowly, letting levels creep up. With sirolimus that could mean more of its known side effects, like mouth sores, low blood counts, infections, high blood sugar, or swelling.

I want to be honest about the strength of this, though. It's only theoretical and based on lab and animal work, never confirmed in humans, so I wouldn't lose sleep over it. Still, because sirolimus is a cancer drug with a narrow safety window, please run any atractylodes product by your oncology team or pharmacist before starting.
