Voxelotor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Beet interacts with.

Theoretically, beet might increase the levels of CYP3A4 substrates.
In vitro research suggests that betanin, the major pigment in beet, competitively inhibits CYP3A4 in a dose-dependent manner similarly to strong CYP3A4 inhibitor ketoconazole.

Here's the short version for voxelotor (Oxbryta): your body clears this medicine using a liver enzyme called CYP3A4. Lab studies hint that the red pigment in beets (betanin) may slow that same enzyme down. If that happened in real life, voxelotor could build up a bit and you might notice more of its usual side effects, like headache, nausea, stomach pain, or a rash.

That said, I want to keep this in perspective. This is theoretical, based only on test-tube research, not on people taking beets and voxelotor together. Eating beets as a normal food is very unlikely to cause trouble. Concentrated beetroot supplements are a bigger unknown. Before adding one, it's worth a quick chat with your pharmacist or doctor.

Oxbryta (voxelotor) is approved to treat sickle cell disease (SCD) in adults and in children as young as 4 years old. It works by targeting the underlying problem in sickle cell disease — the abnormal hemoglobin that causes red blood cells to take on a rigid, sickle shape.

This approval was granted under the FDA's accelerated approval pathway, based on the drug's ability to raise hemoglobin levels. Continued approval may depend on results from confirmatory clinical trials showing broader clinical benefit.
