Voxelotor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Your body clears voxelotor (Oxbryta) mainly through a liver enzyme called CYP3A4. Ceylon Leadwort contains plumbagin, which in test-tube studies appears to slow that same enzyme down. If that effect carried over into people, voxelotor could build up in your blood a little more than usual, which might mean a higher chance of side effects like headache, nausea, diarrhea, belly pain, or feeling tired.

The key thing to know: this is only based on lab research, not real cases in humans, so it stays theoretical for now. Still, since voxelotor is an important medication for your sickle cell disease, I'd run any Ceylon Leadwort use past your pharmacist or doctor first rather than guessing.

Oxbryta (voxelotor) is approved to treat sickle cell disease (SCD) in adults and in children as young as 4 years old. It works by targeting the underlying problem in sickle cell disease — the abnormal hemoglobin that causes red blood cells to take on a rigid, sickle shape.

This approval was granted under the FDA's accelerated approval pathway, based on the drug's ability to raise hemoglobin levels. Continued approval may depend on results from confirmatory clinical trials showing broader clinical benefit.
